WHAT IS A CARE REVIEW?

The partnering agencies in System of Care (Local Management Entity, Department of Social Services, Department of Juvenile Justice and Delinquency Prevention, School System, Guardian Ad Litem & Partnership for Children) support the concept of Child & Family Teams (CFT) to ensure that best practice services are available to children, adolescents and their families within their home community.  A System of Care approach removes agency silos and enables partnering agencies to operate within the model of “one family one team one plan” to focus on problems and prepare a plan of action.

These partnering agencies strive to fulfill certain principles when youth and their family need intensive treatment including:

  1. Matching the needs of youth with what is understood to be best practice or evidenced-based services in the field of mental health and substance abuse
  2. Maintaining and strengthening the youth’s connection to the community
  3. Providing youth with services that match their needs in the least restrictive setting while valuing the preferences and culture of the family
  4. Supporting the family in meeting the needs of their child.

The Care Review Team is a representative body from partnering agencies charged with ensuring that these principles become reality so that youth and their families are supported within the community.

Child and Family Teams schedule a Care Review in the following situations:

1.  Out of Home Placement:      
The Child & Family Team will present their reasons at Care Review when considering out-of-home placement for a period longer than 30 days.  Care Review also serves as the approval body for state-funded Psychiatric Residential Treatment Facilities (PRTF), Wright and Whitaker Schools. 

Should emergency out of home placement be needed, a Child and Family Team should be convened as soon as possible and appropriate placement action taken.  The case should then be scheduled for Care Review at the next available time following standard referral procedures.

2.  Discharge or Transition Planning:

When the youth is returning home following an out-of-home placement lasting over 30 days the Child & Family Team should meet to prepare a transition plan that ensures services are in place.  This transition plan will then be sent to Care Review.

3.  Child & Family Team Requests Assistance:

The Child & Family Team may seek assistance from the Care Review team when the team needs assistance in further planning.
